We are providers for all of the service area's major insurance companies; including Medicare.  See below for a non-inclusive list of those companies, and be sure to ask if you do not see your insurance company listed.

Our office requires payment of deuctables and co-pays at the time that services are rendered. Please be prepared to make these payments on presentation to the clinic.

Many insurance companies have requirements regarding either or both visits for the clinic and procedures. Some companies (especially HMOs) require a written referral request from your primary care physician. Our staff will do our best to help make sure that your requirements are met, but sometimes we may ask you to assist us in obtaining these referrals to expedite your visit and avoid any confusion regarding your coverage.

We do make it a policy to see any patients referred; regardless of insurance status.  Our office manager can speak to you regarding payment plans if you are not insured and need some assistance in this matter. It is important that you let the staff know this when you make your appointment.
